A unique resource for young people experiencing mental health problems who can be safely placed in a residential care setting
Family Care Associates has developed a partnership arrangement with Affinity Healthcare that now allows us to safely and effectively manage young people experiencing mental health and psychological problems in a community setting.
In 2008 we launched what we believe may be a unique jointly-managed Children’s Home that, in essence, will allow young people currently being treated within a secure mental health setting or long term child and adolescent unit to be reintroduced back into the community in a planned and coordinated manner with a continuum of clinical and social support. Having made this transition young people may either be returned to their birth family, foster care, another children’s home or independent living as directed by their Care Plan. In some cases it may be that this children's home, with its specialist clinical support, will provide a suitable alternative to inpatient care and treatment.
Affinity Healthcare operates a range of mental health services for young people, including ‘open acute’, secure and low secure provision and this new resource will complement the existing care pathway for young people under their care. The aim is to make the step of living back in the community in a carefully planned and ‘seamless’ manner, with continuity of care and support reflecting the shared philosophy of the two service providers. As part of the development process for this initiative Affinity Healthcare and Family Care Associates have successfully collaborated in supporting the transition for a number of young people from the hospital setting to residential care provided by Family Care Associates. This work, which has been successful, is and will continue to be fundamental in our continued development of this service.
We appreciate that many of the young people who may benefit from this service will already be in hospital and we are happy to discuss either potential admissions or meet to consider if the our partnership arrangement might provide an alternative option to plans that appear to be 'stuck'.
